ITLS-Conference-Exhibitor-Header-RegOnline

To request your continuing education certificate, all conference participants must complete the Overall Evaluation & CE Attestation online at:

http://ITLS.2013ConfOverallEval.sgizmo.com/s3/

If you do not complete this online form, you will not receive a CE certificate. The online form can be completed on your laptop, tablet or smartphone onsite at the conclusion of the conference – the most convenient way to ensure your CE attestation is fulfilled when you leave.

For the most timely processing of your certificate, please complete the online form by November 30 . Individualized CE certificates will be created based on this information and will be e-mailed to participants within 6 weeks after the conference.

Additional Requirement for Earning CNE & CEH Credit For Nurses & All EMS Personnel:

ITLS is required to collect documentation related to participation in continuing education activities. Attendees seeking CNE or CECBEMS credit must have the room monitor sign the Continuing Education Verification Form for each session attended.

Please refer to the bright blue form in your participant materials for additional information. This form should be submitted to ITLS staff at the end of the conference. It will be used in conjunction with your online Overall Evaluation & CE Attestation to issue your certificate.

If you do not complete BOTH the hard copy CE Verification Form and the online Overall Evaluation, your CNE or CECBEMS CEH certificate cannot be issued.
